ProcedureEdit
- Complete the proof of age card application form. You can download the form or pick one up from a Driver and Vehicle Services Centre or regional Agent. If you would like a copy posted to you, you can contact Department of Transport Driver and Vehicle Services. - Attend a Driver and Vehicle Services Centre or regional Agent and bring all the required documents.
- Application will be processed and card manufactured.

Required DocumentsEdit
- completed application form
- one primary and one secondary proof of identification documents - one of which must contain your signature
- your photograph
Primary IdentificationEdit
- An Australian passport (expired no more than two years);
- An original birth certificate or a certified copy;
- A photographic drivers license issued by any Australian licensing authority (expired no more than two years)
- A current overseas passport;
- A photo identity card for police or defense force personnel (this excludes identity cards issued to civilians);
- A current Document of Identity issued by the Department of Foreign Affairs and Trade;
- An Australian citizenship or naturalization document or immigration papers issued by the Department of Immigration and Citizenship;
- A consular photo identity card issued by the Department of Foreign Affairs and Trade;
Secondary IdentificationEdit
- A current entitlement card issued by a Commonwealth Government department (for example, a Medicare card or a Pension card);
- A current credit card or ATM access card issued by a financial institution;
- A passbook or account statement from a bank, building society or credit union not more than 12 months old;
- A telephone, gas or electricity account not more than 12 months old;
- A water or local rate notice or land valuation notice not more than two years old;
- An electoral enrollment card or other evidence of enrollment not more than two years old;
- Armed Services discharge papers not more than 12 months old;
- A State or Federal Government employee photo identity card;
- A certificate or statement of accomplishment or enrollment from a recognized educational institution;
- A letter, not more than 12 months old, from the principal of a recognized educational institution.

InstructionsEdit
- When applying for a Proof of Age card, you must attend a Driver and Vehicle Services (DVS) Centre or regional DVS Agent in person and provide one primary and one secondary proof of identity document. One of these identity documents must contain your signature.
- You must be aged 18 years or over to apply for a proof of age card
- You must provide one primary and one secondary proof of identification documents - one of which must contain your signature (accepted proof of identification documents are provided in the application form below)
- A student ID card is not an acceptable form of identification
- You must supply a Western Australian residential address
- It is an offence to give false or misleading information. Penalties will be enforced under the Liquor Control Act 1988 and Liquor Control Regulations 1989. Fraudulent applications will be referred to the WA Police
- There is no exemption from the photograph requirement. All proof of age cards issued must display a photograph of the card holder
