Mail-In Services

1. Complete an order form.
2. Send the completed form along with your check or money order - $20.00 per certificate. Do not send cash or credit card information.

Apply Online

The Washington State Department of Health, Center for Health Statistics, does not accept credit cards for online orders of vital records. However, for your convenience, you can order records online through an independent company that they have partnered with to provide you this service: VitalChek Network, Inc.

Go online and simply follow the steps to place an order. An additional fee is charged by VitalChek for using this service and all major credit cards are accepted.

By Phone

1. Make sure you have the following information ready with you before ordering, credit card number, credit card expiration date and address where you would like your certificate to be sent to. If you are requesting UPS Next Day Air delivery (for an additional fee) you must also include a street address. UPS Next Day Air cannot deliver to a post office box.
2. Place an order by calling (360) 236-4313. Hours of operation are Monday through Friday 9:00 am to 4:30 p.m.

In-Person

1. Use the following address for applying in person : Center for Health Statistics, Department of Health, 101 Israel Road SE, Tumwater, WA 98501.
2. You will need to complete an order form when you apply. You may download, fill out, and bring in the order form to the counter at the address above. You can also get these forms at the counter in the office and fill them out there.

The divorce must be registered with Washington State Department of Health.

Fees for Each Certificate: $20.00 US - (Make checks/money orders payable to Department of Health)
For Expedited Delivery you must add Express Mail, an additional $18.30 US
Federal Express: an additional $15.00 US
To AK/HI/Canada/Mexico, an additional $25.00 US

By Phone

Cost for Over the Phone: One time service charge ( $6.50 VitalChek and $5 expedite) plus $20 for each certificate. The total cost is $31.50 for one certificate and first class mail. Payable by Visa, Mastercard, Discover and American Express.
For Expedited Delivery you must add:

• UPS Next Day Air, an additional $18.50 US
• UPS AK, HI & PR, an additional $25.50 US
• UPS Canada/Mexico, an additional $26.50 US

Processing Time

• Mail: Your certificate will be shipped within 4 weeks after payment is processed
• Online: Your certificate will be shipped within 2 business days
• Phone: Your certificate will be shipped on the next business day
• Walk-In: Same day. Orders after 4:00 p.m. may be processed next business day

Instructions

The Washington State Department of Health issues certified copies of vital records (or certificates) for births, deaths, marriages and divorces that took place in the state of Washington.

The office have divorce certificates for divorces taking place from January 1, 1968 to the present. They do not have copies of divorce decrees.

For divorces that took place in the last two months OR before January 1, 1968, or for copies of divorce decrees, contact the county clerk where the divorced was finalized.

The Customer Service Counter is open to the public 8:00 a.m. - 4:30 p.m. Monday through Friday, except state holidays. For further information please call (360) 236-4300.
