Uttar Pradesh - Apply for State Disability Pension

ProcedureEdit

Under this scheme, financial assistance is provided to severe and multi-disabled persons who are living below poverty line.
Apply In-Person:

  1. To apply for this scheme, the eligible person has to submit two copies of duly filled application form along with required documents to the concerned District Magistrate office of the concerned District
  2. Link for contact details: link
  3. Obtain the application form from Jan Suvidha Kendra/ Lokvani/ Internet OR write on a plain paper
  4. After scrutiny, the application is forwarded to the concerned authority for approval.
  5. Following the approval, pension will be remitted monthly to their bank accounts directly through E-Payment.
  6. In case of rejection of application, the applicant is duly informed.

Apply Online - Divyang Pension

  1. Please follow the below link to apply online: Link
  2. In the given page please select “Apply online” option under Divyang Pension to reach next page.
  3. Here plese select “New Entry Form” option.
  4. User will be provided with “application for Department of Disability Empowerment Uttar Pradesh Pension” form. Please fill in the form as appropriate against the prompts. Upload applicable documents against prompts. At the end click “Save” button.
  5. A registration acknowledgementslip with Registration No is generated on screen which can be printed for future reference
  6. Now come back to the Divyang Pension page, Select “Edit Saved Form / Final Submit” option.
  7. In the resulting page please select as appropriate and provide the register number at the prompted space and complete other prompts as appropriate. Please click “Search” button.
  8. Applicant will get the online application form saved and with prompts for entering details. Please provide applicable details and click “Final Submit” button.
  9. Upon Final Submit, the Application form will automatically forward toDSWO/DPO/DHWO.
  10. This application will be processed further.
  11. Applicant shall follow the notification from the department for completing the other requirements as appropriate to avail the scheme.

  • Signature/Thumb impression of the patient countersigned by the Medial Superintendent/CMO/Head of Hospital (with seal)
  • Proof of Age
  • Recent attested photograph showing the disability affixed here.
  • Domicile certificate as per requirement
  • Three passport size photographs of the applicant.
  • Aadhaar card.
  • Bank account details
  • Mobile number.
  • Passport size photograph of applicant (please take 5 copies).
  • Any other document to claim benefits offered by respective state government as advised by authorities.

EligibilityEdit

  • Who have completed 18 years of age and has minimum 40 percent of disability.
  • Resident of Uttar Pradesh and presently living in Uttar Pradesh.
  • Individuals that are receiving pension/grant/aid under old age pension, widow pension, Samajwadi pension or any other scheme and individuals receiving free sustenance in government institutions/shelters will not be eligible for this scheme.
  • Rs.500/- per beneficiary per month that will be changed with the valid rates from time to time by the government.
