Apply by Mail:

  1. To Get Permit or License  to Make and Register a Firearm, the applicant has to approach the Bureau of Alcohol, Tobacco, Firearms, and Explosives link
  2. An application form will be provided to the applicant by the concerned officer. or it can be downloaded from the following link
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the necessary documents along with the application form to the completed mailing address.
  7. The mailing address is mentioned in the “Office Locations & Contacts” section of this page.
  8. The applicant has to pay the prescribed fee as per the authority’s request.
  9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  10. Once the process gets completed, the officer will contact the applicant through email or phone.
  11. This entire process can be completed within 30 days or as per the timeline advised by the respective department authority.

  • Are any of the firearm(s) connected to another multiple sale?
  • If you sold these firearms at a gun show or other qualifying event, identify the event and provide a complete address of the event
  • Rifles Sold or Otherwise Disposed of to the Same Unlicensed Person at One Time or During Any Five Consecutive Business Days
  • Transferee's Name
  • Residence Address
  • Sex
  • Ethnicity
  • Race
  • Identification Number
  • Type of Identification
  • ID State
  • Date of Birth
  • Place of Birth
  • If the buyer of the firearms listed in item 4 is a person authorized to act on behalf of a corporation, company, association, partnership, or other such business entity, you must record the following
  • Additional Information Relating to the Transfer of the Firearms
  • Name of Employee Filling Out This Form
  • Date This Form Was Completed



Need for the DocumentEdit

  • Multiple sales occur when a licensed dealer or pawnbroker sells or otherwise disposes of more than one semiautomatic rifle capable of accepting a detachable magazine and with a caliber greater than.22 (including.223/5.56 caliber) to an unlicensed person at the same time or within five consecutive business days.
  • When a federal firearms licensee (FFL) sells multiple firearms to the same buyer within a particular period, the Gun Control Act (GCA) of 1968 mandates that the FFL send a report to ATF.
  • The ATF investigates probable weapons trafficking cases based on information acquired from several sales transactions.
