How to apply:
Offline process:
- Report to a police station near you and notify them about your lost or stolen national ID and obtain a police report.
- Visit the National Identification and Register Authority Office (NIRA) with all the required documents and the recommended fees and request for the application form 11 or download the form via (NIRA) website using a browser on any device : Application
- Fill in all the required information accurately and sign the document. Once done, the next step is to process your payment slip via the Uganda Revenue Authority(URA) website portal (check below)
- Upon printing out the acknowledgement payment receipt via the URA portal applicant should Visit the bank and make your payment
- Gather all the documents required for the application and visit the NIRA office near you for processing
- The NIRA staff will then take your Bio-metric detail, your details will be verified, purpose acknowledge and application received. You will be given a notification number and also be told on the date and location to pick your new national ID
Online process:
- Applicant should then Visit the official Uganda Revenue Authority (URA) website for the payment process : ura
- On the home page click on e-services, you will be redirected to the next page then click on Payment Registration in the Payments section.
- Select DT as the Payment Type and choose Other NTR.
- Select NIRA in the Ministry section and Miscellaneous Payments as Department.
- Remember to select Replacement of lost ID as the TAX HEAD option.
- Fill the rest of the form and validate all the information are filled accurately and print your acknowledgement payment receipt.

InstructionsEdit
- Report the loss of your national ID card to the police station in the area in which you lost the card, and if you do not remember where exactly, report to the nearest station. You should get a replacement request notice from the police which you will use in the process of contacting NIRA.
- If it is not, you will have to download NIRA application form 11 for lost identity card and fill it in accurately. Then, visit the URA web portal, register for a Payment Reference Number and pay the fees (UGX 50,000) to the bank.
- After this, go to NIRA offices, submit in the form, police report and confirmation of payment slip. It is from here that you should be told of the next step of securing your replacement.

- The identification card is used to prove one is a Uganda citizen, and all Ugandan citizen of 18 years and above are required to have a valid ID card

- For replacement of a lost or stolen national ID, you’ll be charged UGX 50,000. (The money is paid at any URA revenue collection bank such as Bank of Africa, Diamond Trust Bank, Stanbic, Barclays, Orient, etc. [Remember, the fees are exclusive of bank charges].)

There are many situations where you need to prove that who you are. The identity card will allow you to do this in number of ways;
- Proving Age
- Traveling abroad within the East African Region (as per the signed protocols allowing for the use of the National Identity Card across the region)
- On day to day basis for identification to law enforcement officers.
- Accessing financial services e.g. Opening a bank account.
- Processing land transactions
- Confirming right to work.
- Accessing public services for which citizens are entitled.
- Identifying you as a citizen for purposes of voting.
- Registering for a SIM card
