Trinidad and Tobago - Apply for CDAP Smart Card

- To apply for a CDAP Smart Card (My TT Card), you'll first need to have an electronic birth certificate. Issuance of the card is based on the new birth certificate's electronic identification.
- To get an electronic birth certificate: Apply for a new birth certificate through TTPost. Go to any TTPost outlet and ask for a Trinidad and Tobago Birth Certificate Form. Fill it out and post the completed form.
- Your new birth certificate will be mailed to you within two weeks if there are no queries or problems with your application.
- Go to your pharmacy and show them your birth certificate and your current CDAP Prescription Forms.
- The staff will give you a MY TT CARD Application Form to fill out.
- Submit it to the pharmacy.
- Ask the staff when can you come back to pick up your MY TT CARD.
- You may use your card immediately once received.

Chronic Disease Assistance Programme (CDAP)
The CDAP provides citizens with free prescription drugs and other pharmaceutical items for these following health conditions:
- Diabetes
- Asthma
- Cardiac Diseases
- Arthritis
- Glaucoma
- Mental Depression
- High Blood Pressure
- Benign Prostatic Hyperplasia (Enlarged Prostate)
- Epilepsy
- Hypercholesterolemia
- Parkinson's disease
- Thyroid diseases

CDAP Patients
The CDAP programme can be availed by all citizens of Trinidad & Tobago. CDAP cardholders are provided with free prescription drugs for the treatment of Diabetes, Hypertension and other cardiac Diseases, Glaucoma, Asthma, Arthritis, Depression & Benign Prostatic Hyperplasia (Enlarged Prostate).
To ensure that you get timely service from CDAP, please don't forget that:
- Persons over 18, please walk with your National Identification Card when you visit your doctor or clinic.
- For children (18 years or younger), the new computerised Birth Certificate (which provides a unique personal identification number (PIN) should be presented.
- Once a prescription is required, the doctor will fill out three (3) copies of the CDAP prescription form.
- The doctor will keep one copy and give you the other two (2) copies. Take both copies along with your ID to the pharmacy. The person who is picking up the prescription must walk with his/her ID and the patient's ID.
