Through Meeseva Centre

  1. Applicant shall approach the meeseva centre to apply.
  2. Link for contact: Link please select the menu “Meeseva Centres” on the left part of the screen and select the sub option “Authorised service provider” sub option to get next page for list of service provider.
  3. Go to the service centre.
  4. Obtain 'Birth Certificate Update/correction Form' from the respective counter or write in a paper to apply Or download from the following link: Form Link.
  5. Click on the “Application Forms” option and select the “Demarcation Application Form” Under the Revenue option.
  6. Please complete the application form and attach the required documents and submit it to the operator.
  7. The submitted application will be processed by the operator. Applicant will be provided with transaction id. This will be useful to track the status of application.
  8. This application will be processed further by the respective department.
  9. Once the process is over, applicant gets notification to collect the certificate.
  10. Use the transaction id to obtain certificate from the office as per notified period.
Note: This changes will be published in a local newspaper and with the respective state official Gazette publication.

Mother’s name needs to be changed from <Present_Name> to <Corrected_Name>.
