Procedure

Apply In-Person:

1. Any applicant who are willing to register their facility as a Private Medical Institution Licence, they should approach the Ministry of Health, Zanzibar contact link
2. The applicant should get the form from the office and fill it up with all the necessary information in it.
3. Documents that are needed for completing this procedure are mentioned under the “Required Documents” section of this page.
4. The applicant should have both photocopies and originals of all the documents that are needed for completing this procedure.
5. The applicant must attach the documents in either their photocopied form or original form as per the requisition in the application form.
6. Finally, the applicant should submit the application form along with all the respective documents to the concerned authority at the office.
7. The fee for this procedure must be paid as per the authority’s request.
8. After all these procedures are done, the applicant will be provided with the information on how long it will take for them to receive the Private Medical Institution Licence.
9. The applicant can visit the office at the day mentioned by the concerned authorities.
10. If there is no information regarding the licence is provided, the applicant can visit the office to know the status of their application.

Apply Online

1. Online application for getting a Private Medical Institution Licence is available in the following login link
2. Applicant should click on “Login” for opening the page for logging in.
3. If the applicant is an existing user, they can login directly using their registered “Username or Email”, “Password” and by clicking on “Login”.
4. If the applicant is a new user, they must click on “Create Account” for creating a new account.
5. For new account creation, the applicant should provide the following,
   • First Name
   • Middle Name
   • Last Name
   • Email Address
   • Phone Number
   • Position
   • Zone
   • Region
   • District
   • Council
   • Verify Code (changes every time the logging in was done)
6. After providing all these information, the applicant must click on “Submit” for getting their username or email address and password.
7. After successfully getting registered, the applicant can use the “Username or Email Address” and “Password” for logging in.
8. Once the login process is done, the applicant can find a notification which states that the account is successfully activated.
9. On the left side, the option “Facility Requests” is available which the applicant should click on it.
10. On the following portal, the applicant will be provided with the online application form.
11. The first page of the application form will request for basic details, which the applicant must provide and then click on “Next>>” button.
12. On the next page, the applicant should provide more details which are requested in the form about the service provided, infrastructure and other details about the medical institution.
13. Finally, the applicant should click on “Submit” button for submitting the application form.
14. If all these procedures were followed properly by the applicant, they will be informed about the other procedures involved in getting the licence.
15. If the applicant gets through all these procedures, the licence will be provided via post or the applicant can visit the office to get the licence by visiting the office right after getting SMS or Email.
16. The processing time for this procedure will be mentioned via E-mail or SMS.
