Apply Through CSC

  1. To Apply for CM (Chief Minister) Health Insurance Scheme, the applicant has to approach the nearest CSC/ KISOK centres in their area– link

Note: The applicant has to select state, district and block name from the drop-down menu to find nearest office to them

  1. An application form will be provided to the applicant by the concerned officer or the applicant has to download the application form from the following link – link
  2. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  3. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  4. The applicant has to attach photocopies of the required documents along with the application form while submitting it.
  5. Submit all the documents with the completed form to the concerned officer.
  6. The fee for this procedure is free of cost.
  7. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  8. All applications are processed within a reasonable time, having regard to the particular circumstances of each application, including the completeness of information and documents submitted to the authority by the applicant.
  9. The authority may seek additional information from an applicant if this is necessary to assess the application. The authority does not need to deal further with an application until the applicant has provided this additional information.
  10. Biometrics (Eye & Fingerprint Scanning) and Photo verification was done at verification centre.
  11. Upon verification and biometric scan, an e-card will be issued shortly.
  12. This entire process can be completed within 7 days or as per the timeline advised by the respective department authority.

EligibilityEdit

To eligible for Chief Minister Comprehensive Health Insurance Scheme, the applicant must be

  • Resident of Tamil Nadu
  • Name should be displayed in Family Card/Ration card
  • Family income less than 1,20,000 per annum.


FeesEdit

  • This is a free service and therefore no fees is applicable


ValidityEdit

  • The Chief Minister’s Comprehensive Health Insurance Scheme is valid for one year and then needs to be renewed by the policyholder. A maximum coverage amounting to ₹5 Lakhs is provided to all family members.
