Apply In Person

  1. To Request for Amendment of Personal Particulars, the applicant has to approach the MINISTRY OF MANPOWER Link
  2. Applicant has to get the form from the respective office or download the following Link
  3. Applicant has to fill-in all the necessary details in the form, incomplete application are not accepted.
  4. Make sure that the applicant has attached all the documents mentioned in the “Required Documents” section of this page.
  5. Submit completed form and documents in the following Link
  6. After submission the authority will review the application and contact the applicant 7 working days.
  7. Fee for this procedure has to paid as per the authority request.
  8. The applicant check the status via the following Link

Passport Number and/or Passport Expiry Date

  • Pass holder’s passport
  • Documentary proof, if there are other changes to the information on the passport biodata page (e.g. slight changes Nationality in name)


  • Name
    • Pass holder’s passport
    • Deed poll of change in name or
    • Marriage certification that shows change in name after marriage


  • Date of Birth/Country of Birth/Country of Origin
    • Pass holder’s passport
    • Birth certificate showing date of birth, country of birth and country of origin


  • Marital Status
    • Pass holder’s passport
    • Documentary proof of change in marital status (e.g. marriage certificate, divorce papers)


  • Old Malaysian Identity Number/New Malaysian Identity Number/Malaysian Identity Type
    • Pass holder’s passport
    • Front and back copy of new Malaysian Identity Card
