  1. To Obtain Poison Holder License, the applicant has to make the application in Health science Authority’s (HSA) Pharmaceutical Regulatory Information System (Prism) portal Link
  2. The applicant has to click sing pass login or HSA pin. If the applicant clicks singpass login it ask us to login with either Singpass app or Singpass Id number and password QC.
    Note: If you don’t not have an account you can register by clicking on register for singpass, read the information marked 1 and 2 and agree to the terms and condition and click on register now, no the next page entry your personal information and submit. You will be issued with you Sing Pass ID number and password.
  3. The applicant has to provide particular of the applicant, particular of the company and particular of the warehouse.
  4. Make sure the applicant has a required documents that are mentioned in the “Required Documents” section of this page.
  5. After successful submission the applicant has to pay the prescribed fee via credit or debit card.
  6. After the successful payment, the applicant will receive the license within the stipulated period of time.

  • Applicant’s NRIC
  • Current layout plan for the premises, specifying the storage area(s) (Mandatory)
  • The layout floor plan will need to have the following information:
  • The full address of the warehouse
  • The dimensions (length and width) of the warehouse
  • Indication of various storage areas, e.g.receiving bay,quarantined product area,released product area,rejected product area, returned product area,recalled product area, outgoing staging area, etc.
  • Good Distribution Practice Standard Operating Procedures
  • Good Distribution Practice Records or Recording Templates

  • Any person who intends to obtain a poison licence are eligible to make this application.



FeesEdit

  • Application for Form(A) Poisons Licence (FAPL) - $268
  • Licence fee of FAPL for the first year - $103

  • The validity of the License is for one year.
