Singapore - Apply for Copy of Death Certificate/Extract

Death Certificate is an official document that issued by the local county health service management department to prove that someone died. When one dies, all the hospital records are sent to the county health records offices. The date of death is recorded and all the other hospital details. If you want the Death Certificate of your loved one, you just need to go to the county offices, fill an application form and it will be processed.
NOTE: All deaths that have occurred in Singapore or to a Singapore residence living abroad must be register to the relevant authorities for a death certificate to be processed and produced. And if the certificate is damaged or lost then you will have to apply for a Death extract
Apply in Person for a Death Extract
- Submit your application at service center to address provided below in office and contacts section.
- Proceed to obtain a Q-number and Extract requisite application form.
- Complete the requisite application form and wait to be called.
- Once you are called submit the form with all relevant documents to the attending officer who will verify your form and approve your request.
- Pay the required fee
- Obtain receipt for collection after the application is processed
Apply Online
- You can apply for a death extract online using e-Service.
- Open the hyperlink above and select either one of the groups stated.
- Once the page is open select apply death extract.
- This will lead you to the next page where you will have to log in if you have a user account or register.
- Log in and fill the application form
- Submit the form electronically and send payment to the Registry of Births & Deaths.
- The processing time is approximately 1 week upon receipt of payment.
Application Abroad
- You can make the application through online or send a proxy to make the application on your behalf.
Application by a Proxy
If you are living abroad or in Singapore and you cannot make the application by yourself, you can select a representative to make the application for you provide that he/she meets the requirements for representation. When you authorize someone to collect the certified death extract on your behalf. The following must be produced:
- Letter of authorization stating the collector's name and identification document number.
- Collector's identification document and official payment receipt.
- Without official payment receipt, the collector must furnish the application reference number.

- Certified copy of applicants official identification document (e.g. passport, driver's license, etc) (if the applicant is not a Singapore Identity Card holder)
Additional Documents: If the extract will be collected by a proxy of the applicant, the following documents are needed:
- Letter of authorization stating the proxy's name and identification number
- Proxys identification document (IC/passport)
- Official payment receipt or the application reference number

FeesEdit
Upon approval, the following fees are payable:
- S$40 for an extract
- S$40 for a one-month search period
- S$40 for every additional month or part thereof.
The search fee is in addition to the extract fee that needs to be paid after the record is found. It is non-refundable.
Payment can be made using either:
- Visa or MasterCard credit/debit card,
- American Express (AMEX) credit card,
- Internet direct debit (DBS/POSB, OCBC, UOB, Standard Chartered Bank and Citibank internet banking accounts in Singapore)

- Applications will only be processed upon receipt of the payment.
- Approximately 1 week. If death registration number is not available, the processing time may be longer as additional effort is involved to search for the death records.
The general processing time for a death extract application is 4 working days. Death extract(s) will be dispatched by post to the applicant. The application processing time excludes local postal delivery time, which generally takes 2-3 days.
For a death record search, you will be contacted once the search results are available.

To process a death certificate
- First, obtain the death clarification from the hospital when the person died. This is a true verification that the person died from a certain illness. Once the hospital provides all this data, you can then go to the county health managing department, and you will be given a form to fill. You need to be a relative or a family member to the deceased for these processes to be conducted without any doubt. Once you have filled everything, the Death Certificate will start to be processed immediately.
Contents That Are Included in the Death Certificates
- Full name of the deceased person just like it appears on the National ID or passport card, sex of the deceased and the names of the parents of the deceased, the full date of birth of the deceased and the month, date and year of death should be filled in the form to ensure that the number of years of the deceased are calculated perfectly. Further the place birth of the deceased, including the city, address and street number. Include Your Relationship to the deceased and why you are looking for the Death Certificate
Death Registration Documents Required
- Certificate of Cause of Death issued by doctors or Authorised Officers (i.e. Forensic Death Investigators) from the Ministry of Health
- Deceased's identification documents (e.g. IC & Passport) for cancellation
- Informant's identification documents (e.g. IC or Passport)
To register a death:
- The informant must produce the Certificate of Cause of Death and other supporting documents (e.g. the deceased's NRIC, passport or citizenship certificate)
- After documents produced are verified, the Registration Officer will print the Death Certificate
- The informant must verify the particulars on the Death Certificate and sign the Death Certificate
- The Death Certificate is issued.
