ProcedureEdit
- Go to the nearest Access PEI Centers.
- Present the required documents.
- Fill out forms. (An application form is available online. Check the link below.)
- Pass accomplished forms.
- Pay corresponding fees through accredited payment systems.
- Present valid IDs (and authorization) in claiming/receiving the certificate.

- Change of name for minors: $115.90
- Change of name registration outside the province: $25
- Change of name certificate: $35

InstructionsEdit
- A minors guardian, who has a lawful custody of a minor (who have resided in PEI for at least three months), may apply for a name change in behalf of the minor applicant.
- A written consent from the lawful guardian or parent who has lawfully accessed to the minor whose name is to be changed must be provided.
- The consent must be personally written by the minor whose age is 12 or older.

- Applicants (person whose name is to be changed) present and proposed name in full
- Date and place of birth (person whose name is to be changed)
- Details regarding the persons (person whose name is to be changed) previous change of name
- Applicants full name, address, place of ordinary residence during the application, length of stay in the province
