Poland - Obtain an Emergency Travel Certificate
ProcedureEdit
The applicant for an ETC must send an application form together with certified photocopies of any available proof of identity and nationality to the authority designated by his/her country of origin.
The EU country issuing the ETC collects from the applicant charges and fees equivalent to those it normally levies for issuing an emergency passport. An applicant who is unable to cover other local related expenditure will receive, where appropriate, the necessary funds as instructed by his/her country of origin, pursuant to the relevant provisions of Decision 95/553/EC.
It is recommended to get in touch with the consul of the certain country, in which the consul is serving his duty, and in which the requesting party is currently residing (even if, the residence of the consul is located in other country), by telephone (or via e-mail) in order to find out what kind of requirements individual needs to fulfill in order to receive for a mentioned document (Warning: Honorary Consul does not deal with issues in that particular category). Consul will provide the inquirer with practical advices and suggestions regarding the application procedure for the Emergency Travel Document.

- A Letter of Application stating the circumstances surrounding the loss of passport and/or relevant documents.
- A Police Report

Need for the DocumentEdit
The Emergency Travel Certificate (ETC) is a document issued by the High Commission to enable Polish citizens with consular problems or without valid passports travel back to Poland.
If the EU citizen will lose his passport during his stay in the Poland, he is able to report this issue to the representative diplomatic consul of the member state of the European Union in order to receive Emergency Travel.
This decision establishes a standard model for an emergency travel document (ETD), as described in Annex I. ETDs are issued to European Union (EU) citizens for a single journey back to the EU country of which they are a national, to their country of permanent residence or, in exceptional cases, to another destination (inside or outside the Union). The decision does not apply to expired national passports; it is specifically confined to cases where travel documents have been lost, stolen or destroyed or are temporarily unavailable.
Information which might helpEdit
It is also vital to acquire information concerning the departing procedure from ones current temporary residence on the basis on aforementioned certificate (without regard for, whether the entrance to this country was on the basis on visa entry, or visa-free entry) and all information regarding ones stay in the transit zone during the onward journey.
In case of lack of sufficient knowledge in that field please contact consul to obtain relevant information.
You should pay special attention to your return itinerary, as the Emergency Travel Document is only issued for a single journey in order for individual to return to the Member State of the European Union).
Other uses of the Document/CertificateEdit
