Apply Online

  1. To Apply for PhilHealth MDR (Member Data Record), the applicant has to make the application online via the following link
  2. Look for “Membership Portal” Under “Membership” and Click “Register/Login”
  3. Log In to the Account Using the applicant’s PhilHealth Identification Number and Password.
  4. Enter the captcha code provided and then click the “Log in” button.
  5. If the applicant doesn’t have an updated account with PhilHealth yet, click “Create Account” in the lower right corner to register.
  6. Fill out the registration form with the applicant’s details, such as name, birthday, phone number, email address, PhilHealth number, and password.
  7. Click the link sent to the applicant’s email address to activate his/her account. Afterward, he/she can finally log in to his/her account using his/her PhilHealth Identification Number (PIN) and password.
  8. Upon logging in, the applicant will land on the “Member Information” page, where he/she can see details of his/her membership,
  9. Click the “Print MDR” Button on Top of the Member Information Page.
  10. Click the corresponding icon for saving or printing the MDR.
  11. The entire process completes within a day.


Apply By Email

  1. To Apply for PhilHealth MDR (Member Data Record), the applicant has to email their request to Philippine Health Insurance Corporation.
  2. The applicant has to send an email to the following email address to [email protected] with the list of information that are mentioned below
    • Complete name (last name, first name, middle name)
    • Date of birth
    • Present address.
  3. The applicant will then receive a notification with a copy of his/her official MDR.
  4. The entire process completes within a day.
