Apply In Person:
- To Apply for LTO Medical Certificate for Driver License, the applicant has to approach the LTO Central Office link
- Schedule an appointment for the medical examination. The applicant may contact the clinic or hospital directly to inquire about their appointment process.
- The applicant has to carry all the necessary documents that are mentioned in the “Required Documents” section of this page.
- An application form will be provided to the applicant by the concerned officer.
- And the applicant has to get the queue number from the service counter.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Once they called, the applicant has to submit all the documents with the completed form to the concerned officer.
- Undergo the medical examination, which may include a physical exam, eye exam, and blood pressure measurement.
- The medical certificate will be issued on the same day or within a few days after the examination, depending on the clinic/hospital's process.
- Pay the fee at the cash counter and get the receipt for further reference.
- This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.
