Procedure

Apply In Person:

1. To Apply for LTO Medical Certificate for Driver License, the applicant has to approach the LTO Central Office link
2. Schedule an appointment for the medical examination. The applicant may contact the clinic or hospital directly to inquire about their appointment process.
3. The applicant has to carry all the necessary documents that are mentioned in the “Required Documents” section of this page.
4. An application form will be provided to the applicant by the concerned officer.
5. And the applicant has to get the queue number from the service counter.
6. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
7. Once they called, the applicant has to submit all the documents with the completed form to the concerned officer.
8. Undergo the medical examination, which may include a physical exam, eye exam, and blood pressure measurement.
9. The medical certificate will be issued on the same day or within a few days after the examination, depending on the clinic/hospital's process.
10. Pay the fee at the cash counter and get the receipt for further reference.
11. This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.
