Procedure

• Usually when a person is pronounced dead, the doctor records the time of death and then a postmortem is done.
  
• The family of the deceased then forwards these papers to the authority responsible for issuance of death certificates.
  
• They fill an application form and they are issued with a death certificate.

Eligibility

For every expired person his/her record should be reported to the government authorities in order to record the death and later for the issuance of Death Certificate.

Instructions

After a person is pronounced dead his/her family should register the death in order to be granted a death certificate. This is necessary so as to continue with burial arrangements.

Required Information

• Name and Surname of the Deceased and his/her nationality.
• Name of Husband/Father/Mother Occupation/Profession of the Deceased
• Address of the Deceased
• Religion, Sex and Caste of the Deceased
• Date, time and cause of death
• Age of Deceased
• Name and Address of the Person Making Report.
• Date of Report

Need for the Document

Death Certificate is very important and vital document evidence. Death Certificate is required for different purposes, such like property matters, to review the causes of death, proof to be presented in court etc.

Information which might help

Presently NADRA Pakistan has started maintaining Death records; they register and issue a Computerized Death Registration Certificate just like Computerized Birth Certificate or Marriage Certificate.

Other uses of the Document/Certificate

Death Certificate is also required to arrange burial or cremation.
