  • Make checks and/or money order payable to OHA/Vital Records and photocopy of valid photo ID
  • Provide a self-addressed stamped envelope. Print or type your name and address on the envelope using all caps.

  • Valid photo identity
  • Driver's license
  • Passport

If you dont have a valid drivers license, ID card, or passport, send photocopies of three (3) different documents that include both your name and current address. Suggested documents are listed below. If you are mailing your order, make photocopies of the documents and include them with your order form.

  • Utility bill (such as telephone, gas, electric, water, garbage removal) or other bill;
  • Insurance statement, medical statement or paycheck stub;
  • Court document or parole document;
  • Work ID, unemployment statement, food stamp or other benefit card (copy both sides);
  • Valid permit for firearms, fishing, hunting or other license;
  • Vehicle registration, title or insurance statement.

EligibilityEdit

By law, only the biological parents are eligible to order a Commemorative Certificate of Stillbirth for a delivery that occurred after January 1, 1999.



FeesEdit

  • Certified copy: $20.00
  • Additional copy: $15.00 each
By Telephone
  • Orders placed by phone must be paid for with a credit or debit card and are charged an additional $12.50 processing fee.
  • Orders are processed through a third party vendor. The total cost for one record ordered by phone and shipped by regular mail is $32.50.

Processing times vary between two to four weeks after the associated fetal death certificate is filed.

InstructionsEdit

  • Certificates of Stillbirth are available to the biological parents when a fetal death record is on file for a stillbirth that occurred after January 1, 1999.
  • This is a memento certificate, not a legal record.
  • As of February 3, 2010, the fee for this certified memento is $20.
  • You may also order a certified copy of the fetal death certificate for $20 if it is needed for legal purposes.
  • Currently Oregon law requires the filing of a fetal death certificate when fetal death occurs after the fetus reaches 350 grams or 20 completed weeks of gestation.
  • Parents may request the filing of a fetal death certificate when the law does not require one to be filled.
  • You may order the certificate either by mail or in person or by telephone.
  • Attach legible photocopy of current, valid ID.
  • Documents must be dated within the last thirty days and show current mailing address where record will be mailed.
  • If you are the parent and you have no ID or other documents, you will need to have the other parent request the stillbirth certificate or obtain ID before placing an order for the record.
