Ohio - How to Obtain a certified copy of a fetal death certificate

- In Person
- Walk-in to the Ohio Department of Health, Office of Vital Statistics in person to receive a copy of your death certificate on that same day.
- The office is located at 225 Neilston Street, Columbus, Ohio 43215.
- Office hours are Monday through Friday from 9:00 a.m. until 3:00 p.m.
- If you have questions, call (614) 466-2531 for more information.
- By Mail
- Download and completely fill out request form
- Make checks and/or money order payable to "Treasurer, State of Ohio"
- Provide a self-addressed stamped envelope. Print or type your name and address on the envelope using all caps.
- Mail completed request form to:

- Ohio Department of Health
- 35 East Chestnut Street, 6th Floor
- P.O. Box 15098
- Columbus, OH 43215-0098

- Vital records in Ohio are kept by the Office of Vital Statistics, a part of the Ohio Department of Health.
- While prior legislation permitted fetal death certificates only after 20 weeks of gestation, the Ohio Grieving Parents' Bill changed that law.
- The parents must only include a doctor's statement that the woman suffered a miscarriage in order to qualify.
- You may request your certified fetal death certificate by in person or by mail.
- Check or money order should be made payable to Treasurer, State of Ohio.
