Procedure

Apply In-Person:

1. To apply for renewal or replacement of Lost Letter of Exclusion in person, the applicant has to visit the office of the Director General of the National Youth Service Corps.

3. Before proceeding to apply for the renewal/replacement, the applicant must first report the loss to the police and obtain a statement which will speak of the facts of the loss.
4. Then, the applicant must produce that statement in a court and obtain an affidavit from the court.
5. After that, a written request is to be made in the form of an application letter for the Lost Letter of Exclusion.
6. Put all the above mentioned documents together and submit it to the Director General of the National Youth Service Corps, in person.
7. In case if the applicant is unable to arrange these documents, the applicant will have to provide the information mentioned in the “Required documents” section of this page, along with the application letter.
8. After submission, the application and other documents will be forwarded for examination and verification processes.
9. Once the verification and validation processes are over, the application will be approved and the officials will proceed to take required steps to issue the letter of attestation/confirmation.
10. When the document is ready to be collected, the applicant will be notified by the office and the applicant can collect it by any applicable means.

• Any candidate of third category, whose Exclusion letter has been lost, can apply.
• The applicant must possess at least minimum information about the letter, like Issuance number and Certificate number.
