Procedure

Apply In-Person:

1. To apply for Doctor or Dentist General Practice Licensein person, the applicant must visit the office of the Medical and Dental Council of Nigeria (MDCN).
2. The address and contact details of the MDCN can be found in the below contact link
3. Visit the office and obtain an application form to be completed for Doctor or Dentist General Practice License with respect to the requirement from the relevant department. Or click here to download application form
4. Also, obtain the other forms that are required to be completed for the registration process.
5. Complete the forms with appropriate information under relevant sections. Attest the forms with your signature once completed.
6. Once completed filling the application form, make sure that you attached all the documents listed in the “Required Documents” section of this page.
7. Then, submit it in person to the relevant authority at the office. Make the stipulated payment as directed by the officials.
8. After submission, the application and other documents will be forwarded for examination and verification processes.
9. Once the verification and validation processes are over and if the application is found sufficient, it will be approved and the officials will proceed to take required steps to issue the Annual license.
10. When the document is ready to be collected, the applicant will receive a notification from the office through email or phone call regarding the collection and the applicant can collect them by any applicable means.

Apply online

1. To apply for obtaining a Doctor or Dentist General Practise License in person, the applicant has to visit the following online application link
2. Visit the link and enter the login details in the relevant fields and click “Login” button, if you also have an account in the portal.
3. If not, click the “Create new account” button and proceed to create a new account. Click “Click here” under the “New Doctor Account” box.
4. Complete the account creation page, click the captcha and click “Create Account” button. Check you inbox for the user details including password.
5. Click the “Click to login” button in the mail and enter the login details, click “Login” button. If your account has been activated by the administrator, you can enter the portal.
6. If you couldn’t login, it means that the administrator has not yet approved and you should wait for at least 24 hours.
7. After activation, login into the portal and proceed apply for the Doctor license. Locate the appropriate option to apply for the license.
8. Click the option and complete the application form by following the onscreen instructions. Upload the required documents by following the onscreen instructions.
9. Also, pay the required fees for the application processing and license. Submit the form by clicking the applicable button.
10. After submission, the application and other documents will be forwarded for examination and verification processes.
