For an Handicapped Applicant

1. Get the application form BA-207 from your local Motor Vehicle Commission(MVC) office. You can also get this form by calling to a MVC Toll-free number in NJ at (888) 486-3339 and out-of-state can call at (609) 292-6500.
2. Fill the front-side of this form
3. Make a physician complete certification portion
4. Qualify 6 Point ID
5. Pay $6 as a fee
