Procedure

Choose on where you want to obtain the drivers records

By mail

• Submit a notarized and completed Release of Driving Records
• Include the fees.
• Include a self address envelope
• Mail these documents to the following address : Motor Vehicle Division, PO Box 201430, Helena,NT 59620-1430

In person

• Go to the Motor Vehicle Division Office that you belong.
• Submit a completed and notarized Released of Driving Records Form.
• If not notarized, submit a copy of your photo identification duly issued by the government
• After bringing the documents that would show proof of identity with you, pay the required fees.

Apply Online

Make use of the States Driver History Records Online through independent vendor.

Eligibility

• Holder of a validly obtained Drivers license
• Resident of US

Fees

• Online request - A valid credit card to pay the $7.25 fee for each record requested.
• Application in person - Pay the $4 fee for each record requested check or money order only (U.S. funds). Credit cards are not accepted.
• Mail application -Pay the $4 fee for each record requested check or money order only (U.S. funds). Credit cards are not accepted.

Validity

Explain the time until which the certificate/document is valid.
e.g. Birth Certificate Valid Forever
