Eligibility

An applicant may only order copies of his or her own Montana birth certificate or one for an immediate family member (parent, spouse, child (must provide proof of relationship/legal need), legal guardian (must provide proof of relationship) or an authorized representative (must provide proof).

Processing Time

2 to 3 weeks from the date of receipt.(This processing time varies depending on staffing and workload.)

Instructions

• The Montana Division of Vital Records (Montana Vital Records) issues certified copies of Montana birth certificates for events which occurred within the State of Montana.
• Birth Certificates are available for events that occurred within the State of Montana from 1907 to present.
• You may order a copy of a Montana Birth Certificate using one of the following services:

  • Place an order over the phone using your credit card:

  • Place an order by mail.
• Valid Identification is required for the purchase of a Montana Birth certificate.
• Personal check or money order should be made payable to Montana Vital Records.

Required Information

• Full name at birth
• The date of birth
• The place of birth
• Gender
• The mother's maiden name
• The father's name
• Name and address of the applicant
• Phone number, including area code
• Reason for requesting the certificate
• Relationship to the person named on the Certificate

Need for the Document

You may need a birth certificate when enrolling in school, applying for a social security card or for certain legal proceedings. This can prove frustrating when you do not have a copy immediately on hand.
