ProcedureEdit

  1. Go to your designated Office District.
  2. Submit the required documents.
  3. Pay $4.00 which is valid for two (2) years.
  4. You must be a holder of the learners permit for at east twelve (12) months so that then you will be allowed to take the driving test.
  5. Once granted, this learners permit will only allow you to drive a car, with the company of an instructor. After reaching 15 years old, you should upgrade to a regular Learners permit.

InstructionsEdit

  • A completely filled up application duly notarized signatures of both parents, guardians. If with divorced parents, parent having the legal custody of the minor must sign.
  • A social security card or a Social Security Administration printout.
  • A birth certificate duly issued by the Bureau of Vital Statistics or State Board of Health. It must be either an original or a certified copy, complete with a seal.
  • If the applicant is below 18 years old, you must exhibit proof that you are in school by providing a signed Certification of School Attendance form duly issued by your current school. Make sure that the date of issuance is not over 30 days old upon submission.
