The parent(s) are eligible to obtain a certified copy of Birth Resulting in Stillbirth Certificate.
FeesEdit
- First certificate for each record ordered: $16
- Additional certificates for same record: $9
- Delayed Filing Fee: Required if there has been more than one year since the fetal death: $40
- Optional - An Expedite fee is a per-record fee that will place your request ahead of other requests. An Expedite fee does NOT include Federal Express or other special mailing fees: $20
- Optional - Federal Express Mail Service is $16 for most deliveries. Higher rates apply to Saturday or international service and to deliveries to Alaska and Hawaii: $16 (per order only)

- You can File a Record and Request a Certificate for a Birth Resulting In Stillbirth at Minnesota Department of Health
- To file a record and request a certificate, provide the all required information about the birth resulting in stillbirth.
- Provide additional applications or documentation as indicated on the application.
- Complete the "Fee worksheet for a birth resulting in stillbirth records and certificates" if you are requesting certificates. There is no fee to file the record for a birth resulting in stillbirth if the fetal death occurred within the last year, unless you request a certificate. There is a $40 fee to file the record for birth resulting in stillbirth if the fetal death occurred more than one year ago.
- The fee worksheet is not required but you may wish to print and complete the fee worksheet to ensure enclosure of the required fees.
