Apply In-Person:
- Pregnant women must contact the ANMs / ASHAs / health workers in their area to receive information about the nearest government health facility where PMSMA services would be provided. Please click on the following link to view your nearest health centre providing the facility Contact Link
- When a beneficiary visits the government health facility designated to provide PMSMA services, the ANM / Staff nurse would register the pregnant woman coming to the PMSMA Centre and provide her a Mother and Child Protection Card and Safe Motherhood booklet.
- The Staff Nurse/ANM will also take the height and weight of the pregnant woman, check her pulse and BP and record the findings and send the mother to the laboratory for diagnostics.
- The report of the investigations would ideally be handed over within an hour and before the beneficiaries are meeting the doctors for further check-ups. This will ensure identification of High Risk status (like anemia, gestational diabetes, hypertension, infection etc.) at the time of examination and further advice.
- In certain cases, where additional investigations are required, beneficiaries will be advised to get those investigations done and share the report during next PMSMA or during her routine antenatal services (ANC) check - up visit.
- During PMSMA, 108 /102 /State owned ambulances/Private empanelled ambulances can also be used for referring those cases identified as high risk.
- Before leaving the facility every pregnant women to be counselled, may be individually or in groups, on nutrition, rest, safe sex, safety, birth preparedness, identification of danger signs, institutional delivery and Post - partum Family Planning ( PPFP ). - To Apply for Pradhan Mantri Surakshit Matritva Abhiyan (PMSMA) the applicant has to make the application via the following link link
- Then applicant has to click on the “Register now” menu.
- Next the applicant has to fill in all the necessary information in the application form which is displayed on the screen.
- Review the applicant and click “Submit”
- The application submitted will be processed and a receipt with the application number will be displayed.
- The applicant is informed through SMS at various stages i.e., Advance intimation about the date of the visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.
Apply through App
- To Apply for Pradhan Mantri Surakshit Matritva Abhiyan (PMSMA) the applicant has to make the application via the Pradhan Mantri Surakshit Matritva Abhiyan app.
- Use the following link to download the mobile app link
- Then applicant has to click on the “Register now” menu.
- Next the applicant has to fill in all the necessary information in the application form which is displayed on the screen.
- Review the applicant and click “Submit”
- The application submitted will be processed and a receipt with the application number will be displayed.
- The applicant is informed through SMS at various stages i.e., Advance intimation about the date of the visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.
Apply Through CSC Centre
- To Apply Pradhan Mantri Surakshit Matritva Abhiyan (PMSMA), the applicant has to approach respective “CSC” centre link
- NOTE: The applicant has to select state, district and block name from the drop-down menu, or the applicant has to click on their respective district to find nearby centres.
- Please visit the centre and consult with the centre operator for your requirement.
- If the operator issues an application form, then collect it and pay the requisite fee. Please complete the application form.
- Now, submit the completed application along with the required documents as per the “Required Documents” section of this page. It is advised to take the documents in original to the kiosk for scanning purposes.
- The operator will process the application online. On successful submission, the applicant will receive an application number as acknowledgement. The applicant will also get a transaction number as a text message on their registered mobile number.
- Applicant has to make necessary payment as centre operator quotes and don’t forget to take the receipt.
- This application will be forwarded to the respective department for further processing.
- ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification.
- The submitted application will be processed by the department. A designated authority (supply officer) will verify the submitted documents and inspect the area of the applicant to clarify the applicant’s details.
- Based on the outcome of the above-mentioned steps and other internal processing, the designated issuing authority will decide on the issuance of the family card.
- The status, viz. Approval or Rejection will be conveyed to the applicant through an “SMS” or applicable norms.
- If approved, the ration card will be hand delivered or sent through post to the address mentioned while applying or issued as per the applicable norms within 60 days or as per the timeline advised.

InstructionsEdit
Filling out the MCP cards at these clinics is mandatory and a sticker indicating the condition and risk factor of the pregnant women will be added onto MCP card for each visit:
- Green Sticker - for women with no risk factor detected
- Red Sticker – for women with high risk pregnancy
- Blue – for women with Pregnancy Induced Hypertension
- Yellow – pregnancy with co-morbid conditions such as diabetes, hypothyroidism, STIs
