Maldives - Apply for New Passport If Lost or Stolen

ProcedureEdit
- Apply to Maldives Police Service for a police report or submit a letter to DIE stating that the Passport has been lost if in Male.
- Call and check whether the DIE has received the Police report from the Maldives Police Service.
- If DIE has received the police report, go to the service counter at the DIE and obtain a token from the token machine and once your token number is displayed, submit the filled application form (IM01) with the necessary documents.
- Make the payment and obtain the payment receipt (the date and time to collect the passport will be included).
- Go to the service counter on the informed date, (after taking a token number) and provide your National Identity Card and the Passport payment receipt.
- The Staff at the counter will request you to give your finger prints twice, once for capturing fingerprint data and secondly to verify the captured image for Electronic passport applications.
- You will also be required to sign the application form for signature verification and as a record that passport has been issued to the rightful owner.

- Filled IM01 form (Dhivehi Passport ah edhey form)
- 01 passport photo (taken not less than 1 month) photo must confirm passport photo standards issued by the DIE. (All registered photo studios are informed of these standards).
- Valid National Identity card (original) or a document issued by the DNR with a photo, and an identity card Number.
- Revenue stamp (worth Rf 10).
- Payment receipt (to collect the passport).
- Police report (original) obtained from Maldives Police Service.

- The amount to be paid for issuing passport is:
- Rf 450 for Ordinary Passport (32 pages).
- Rf 650 for Ordinary Passport (64 pages).
- Rf 300 for Non-Electronic (Ordinary) passport.
- Rf 250 for Temporary Travel Document
- To replace a lost or stolen passport a police report is necessary, and depending on the police report Controller of the DIE has the right to fine the individual with a fee Rf 2000 to Rf 20000 (if the Passport was lost from your negligence).
