Apply In Person:
- To Register for Seaman Record Book, the applicant has to approach the MARINE DEPARTMENT PENINSULAR MALAYSIA link
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- The applicant has to pay the prescribed fee as requested by the authority.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Then the application will be processed further.
- The officer will review the application and may request additional information or clarification if necessary.
- Upon completion successfully, the applicant gets a call from the office regarding the application.
- The applicant has to visit the office and collect the seaman permit.
- This entire process can be completed within 3 – 4 weeks or as per the timeline advised by the respective department authority.

- Address proof – (Driving license, passbook, utility bill, etc)
- Passport (with at least six months validity)
- Seafarer's medical certificate (issued by an approved medical practitioner)
- Proof of sea service (such as a letter from your employer, a contract of employment, or a discharge book from your previous employer)
- Relevant training certificates (such as basic safety training, advanced fire fighting, or medical first aid)
- Four recent passport-sized photographs (with a blue background)
- Payment of fees (as per the prescribed rates)
- Fee receipt
- Any other documents (If required)

- In Malaysia, the Seaman Record Book is also known as the Seafarer's Identification and Record Book (SIRB). It is an official document issued to Malaysian seafarers that contains details of their personal information, training and qualifications, employment, and seagoing experience.
- The Seaman Record Book serves as proof of the seafarer's qualifications, experience, and competency, and is required for employment on board any Malaysian flagged vessel or foreign vessel that enters Malaysian waters.
