Apply In-PersonEdit

  1. Here are procedures on how you can apply for a Makatizen Card. First, you will need to complete a Makatizen Card Application Form. A link is provided under 'Documents to Use' section which you can click on to download the form. You may also ask for a copy of the application form from your respective Barangay Halls.
  2. Prepare both the original and photocopy of these following documents:
    1. Single/Unmarried: Birth Certificate or valid Passport. Must be authenticated or certified true copy issued by the Philippine Statistics Authority or the Local Civil Registry of Makati.
    2. If married: Birth Certificate and marriage certificate or valid Passport. Must be authenticated or certified true copy issued by the Philippine Statistics Authority or the Local Civil Registry of Makati.
    3. Barangay clearance (both the original and photocopy)
    4. COMELEC certification issued May 10, 2016 to present (both the original and photocopy)
    5. One valid government issued ID (see list below).
  3. Inquire at your local barangay hall when will be the next scheduled barangay caravan. You may also proceed to the Makatizen Card Area: Ground Floor, Makati City Hall Building II, Mondays to Saturdays 8:00 AM to 5:00 PM.
  4. Go to the designated area in your barangay or the Makati City Hall where applications are processed.
  5. Submit the completed form together with all the required documents. Original copies of your documents will be returned to you immediately after verification.
  6. After your application is reviewed you will be directed to the data capturing section.
  7. Wait for the announcement from your local barangay office when your card will be available for pickup (usually after 45 days).

Required DocumentsEdit

  • Completed MAKATIZEN CARD application form.
  • Authenticated or certified true copy of the following issued by the PSA (used to be NSO) or the Local Civil Registry of Makati: (both the original and photocopy)
    • Single/Unmarried: Birth Certificate or valid Passport
    • If married: Birth Certificate and marriage certificate or valid Passport.
  • Barangay clearance (both the original and photocopy)
  • COMELEC certification issued May 10, 2016 to present (both the original and photocopy)

    • Social Security System (SSS) card
    • Senior Citizen Card with photo
    • Overseas Workers Welfare Administration (OWWA) ID
    • Seaman’s Book
    • Alien Certification of Registration/Immigrant Certificate of Registration
    • Government Office and GOCC ID, e.g. Armed Forces of the Philippines (AFP ID)
    • Home Development Mutual Fund (HDMF ID)
    • Certification from the National Council for the Welfare of Disabled Persons (NCWDP) with photo
    • Department of Social Welfare and Development (DSWD) Certification with photo
    • Integrated Bar of the Philippines ID
    • Maritime Industry Authority (MARINA) ID (Reference: BSP Memorandum No. 2016-019)
    • Company IDs issued by private entities or institutions registered with or supervised or regulated either by Bangko Sentral ng Pilipinas (BSP), Securities and Exchange Commission (SEC) or Insurance Commission (IC)
    • School ID with Photo (for students who are COMELEC registered voters)

NOTE: Original copies of your documents will be returned to you immediately after verification.

Need for the DocumentEdit

The Makatizen Card program of the Makati City government aims to efficiently provide direct social benefits (example: allowance, scholarship, health and medical) and financial services to the employees,constituents and the general public of the city of Makati.

The Makatizen ID card can be linked to a GCash account which gives the cardholder access to mobile transactions and can be used like a debit card when paying for goods and services.
