Apply In-PersonEdit
- Here's how you can apply for a Makati Health Plus Card (Yellow Card). First, prepare all documents you will need to present or submit when you apply for a Makati Health Plus Card (see list below).
- Get a temporary number for pre-evaluation. (City Hall lobby in front of Makati Health Program (MHP) office). You may also apply at a Makati Health Plus Satellite Station. Check out 'Office Locations & Contacts' section to know where you can apply.
- Submit a completed application form along with all the required documents.
- Your application will be pre-evaluated and you will receive a queque number for the interview.
- You will be interviewed by an officer from the Makati Health Program.
- Applications will be sorted per barangay and will be endorsed to the Makati Action Center (MAC) for home visitation.
- Once approved, a card will be prepared and released to the applicant.

- A registered voter in Makati and actual resident of the City of Makati
- A registered voter in Makati but non-residing in Makati
- A Makati City Government (MCG) employee:

- Casual employees
- Contractual
- Consultant
- A national government employee assigned in Makati City
- Relocatees

One (1) year validity applies to:
- Registered voters but non-residents of the City of Makati
- Makati City Government (MCG)Employees:
- Job Order
- Casual Employees who have rendered service for less than 5 years Contractual
- Consultant
Two (2) years validity applies to:
- National Government Employees assigned at Makati City unless transferred or removed from office before the expiration date of the Makati Health Plus (MHP)Card.
- Relocatees
- Makati City Government (MCG)Employees:
- Casual Employees who leave rendered service for at least 5 years
Three (3) years validity applies to:
- Registered voters and actual residents of the City of Makati
- Makati City Government (MCG) Employees
- Regular Employees
- Casual Employees who have rendered six years and above in service
- Senior Citizens 60-79 years old
Permanent/Lifetime card applies to:
- 60-79 years old and above with the following requirements:
- Certified true copy of the Latest Certificate of Real Property Taxes/Tax Receipt
- Makati Health Plus (MHP) card holder for the past 6 years
- 80 years old and above
- Makati Health Plus (MHP) card holder for the past 6 years
- Retired MCG regardless of residency and COMELEC registration

Need for the DocumentEdit
- The Makati Health Plus Card or Yellow Card program provides government-subsidized health care to cardholders and registered dependents, city government workers and other qualified beneficiaries at the city-run Ospital ng Makati (OsMak), the city’s 26 barangay health centers, satellite laboratories, and three birthing facilities.
- Patients who are Makati Health Plus Card or Yellow Card holders at Osmak with total hospital bill amounting to P5,000 or less are free of charge, while those whose hospital bill exceeds P5,000 will be charged a uniform rate of P500 only, even if the amount reaches six or seven digits.
- Aside from government-subsidized health care, cardholders may also avail free medicines, including maintenance medicines for hypertension, diabetes and other lifestyle diseases, as well as other pharmaceutical products.
