- Application form to Apply for a journalist health card is available in the Department of Public Relations Madhya Pradesh link
- The applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
- The applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
- There is no charge for a Journalist health card.
- Concerned authorities will look into the application form and submitted documents before providing an approval.
- Approval will be provided within 2-3 weeks.
- If the approval has not been given within 3 weeks, applicant can visit the office to know the status of their application.

- Application for Journalist health card is available in the following link and click on “Journalist Login”.
- While registering, applicant has to provide all necessary information in the application form and finally click on “Submit” button.
- Applicant must provide all information necessary for application procedure online.
- Applicant should make sure to upload digital copies of documents (whichever is necessary) along with the application form.
- Finally, applicant has to click on “Submit Application” button.
- There is no charge for a Journalist health card.
- After getting approval from concerned authorities, card will be provided within 2-3 weeks.

- Preference card copy or PPF slip copy
- Form 16
- Old Insert Card Copy (if available)

- AAdhar card and the photos of journalists shall be given in jpeg format not more than 200 kb in pendrive in the I&PR Dept., for uploading.
