Procedure

Apply In-Person

1. Application form to Apply for a journalist health card is available in the Department of Public Relations Madhya Pradesh link
2. The applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
3. The applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
4. There is no charge for a Journalist health card.
5. Concerned authorities will look into the application form and submitted documents before providing an approval.
6. Approval will be provided within 2-3 weeks.
7. If the approval has not been given within 3 weeks, applicant can visit the office to know the status of their application.

1. Application for Journalist health card is available in the following link and click on “Journalist Login”.
2. While registering, applicant has to provide all necessary information in the application form and finally click on “Submit” button.
3. Applicant must provide all information necessary for application procedure online.
4. Applicant should make sure to upload digital copies of documents (whichever is necessary) along with the application form.
5. Finally, applicant has to click on “Submit Application” button.
6. There is no charge for a Journalist health card.
7. After getting approval from concerned authorities, card will be provided within 2-3 weeks.

Eligibility

• The applicant should have at least 15 yrs experience in the field.

• AAdhar card and the photos of journalists shall be given in jpeg format not more than 200 kb in pendrive in the I&PR Dept., for uploading.
