Procedure

Apply In Person:

1. To Apply for a Proof of Life Certificate (Digital Life Certificate), the applicant has to use the following link the Jeevan Pramaan Centre link
2. The applicant has to enter the email ID and enter the captcha code and then click the “I Agree to Download”.
3. The applicant will receive the OTP from the given email ID, enter the OTP and click the “Submit” button.
4. The applicant has to download the application form.
5. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
6. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
7. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
8. After filling in the details, the applicant needs to book an appointment at a Jeevan Pramaan Seva Kendra or a Common Service Center (CSC) to complete the biometric authentication process.
9. The applicant can select a convenient location and time slot for the appointment.
10. On the day of the appointment, the applicant needs to visit the Jeevan Pramaan Seva Kendra or CSC with the Aadhaar card and bank passbook.
11. The applicant’s biometric data (fingerprint, iris scan) will be captured at the center.
12. Once the biometric data is captured, the applicant will receive a confirmation SMS on the registered mobile number.
13. The applicant can then download the Digital Life Certificate from the Jeevan Pramaan website using the Aadhaar number and OTP.
14. There is no fee for a Proof of Life Certificate.
15. This entire process can be completed within 15 - 30 days or as per the timeline advised by the respective department authority.
