Procedure

1. Visit the Public Authority of Civil Information Headquarters at Zahra, ground floor, Citizens' Office.
2. Bring along with you the required documents.
3. Submit the required documents.
4. Fill up service application form.

5. Pay the fee.

• Personal identification (original nationality certificate or passport or driving license)
• For applicants less than (18) years, a birth certificate or passport
• Power of attorney from the Ministry of Justice, in case the applicant does not attend in person

Eligibility

• Applicants or their representatives are required to have a power of attorney from the Ministry of Justice and must attend the procedures.
• A parent in cases of marriage or a custodian in cases of divorce must attend the procedures, for applicants who are less than 18 years old.

Fees

• Replacement for Loss ID Fee: 10 KD
• Individual Details Certificate: 1 KD
