- Applications for replacement of damaged or lost Private Funeral Home institutions Registration Certificate are done online.
Step 1: Obtain a Police Report
- You should start by visiting a police station to report the loss or damage of the Private Funeral Home institution's Registration Certificate. You are to acquire a police report from the police authorities which is a prerequisite to making the application.
- Attach this police report with your application.
Step 2: Apply Online
- Licensed Private Funeral Home institutions can replace their lost or damaged Private Funeral Home institutions Registration Certificate online by visiting the Kenya Medical Practitioners and Dentists Council’s Online Services Portal through the following link: KMPDC Online Services Portal
- Log into the KMPDC Online Services Portal using your “Username or Email address” and your password.
- Upon entering your accounts’ homepage, click on the “Registration” tab in the menu on the left pane to enter the next page.
- On the next page click on the “Replacement ” option to make your replacement application.
- On the replacement page click on the “Click here to make Replacement of Registration Certificate ” button to enter the next page.
- Please attach the police report obtained and click on the “Save & Continue” button.
- This will cause an invoice for payment of the replacement application fees to be generated.
- On the MPDC Invoice page “Click to Pay Now” button to pay for the Replacement of the Registration Certificate.
- Proceed to make the payment using the instructions given online. The Payment instructions will be generated and displayed automatically in the portal. More information about the Replacement fees to be paid is given in the “Fee” section of this page
- Upon making the replacement payment, a payment receipt and the Replaced Private Funeral Home Registration Certificate will be generated online.
- Note: Wait for approximately 10 minutes then refresh the invoice page. After refreshing the page, you should find receipt and license ready to be printed.
- Click on the “Download Receipt “ button and the “Download License” button to download the payment receipt and Replaced Private Funeral Home Registration Certificate respectively.
- Print the receipt and replaced license and keep them for reference.
