Procedure

• Applications for replacement of damaged or lost Private Funeral Home institutions license are done online.

Step 1: Obtain a Police Report

• You should start by visiting a police station to report the loss or damage of the Private Funeral Home institution's license. You are to acquire a police report from the police authorities which is a prerequisite to making the application.
• Attach this police report with your application.

Step 2: Apply Online

1. Licensed Private Funeral Home institutions can replace their lost or damaged Private Funeral Home institutions License online by visiting the Kenya Medical Practitioners and Dentists Council’s Online Services Portal through the following link: KMPDC Online Services Portal
2. Log into the KMPDC Online Services Portal using your “Username or Email address” and your password.
3. Upon entering your accounts’ homepage, click on the “Apply for License” tab in the menu on the left pane to enter the next page.
4. On the next page click on the “Replacement ” option to make your replacement application.
5. On the replacement page click on the “Click here to make Replacement of Annual License for 20xx” button to enter the next page.
6. Please attach the police report obtained and click on the “Save & Continue” button.

   • This will cause an invoice for payment of the replacement application fees to be generated.

7. On the MPDC Invoice page “Click to Pay Now” button to pay for the Replacement of the Annual Health Institution Licence .

   • Proceed to make the payment using the instructions given online. The Payment instructions will be generated and displayed automatically in the portal. More information about the Replacement fees to be paid is given in the “Fee” section of this page

8. Upon making the replacement payment, a payment receipt and the Replaced Private Funeral Home Institution Licence will be generated online.

   • Note: Wait for approximately 10 minutes then refresh the invoice page. After refreshing the page, you should find receipt and license ready to be printed.

9. Click on the “Download Receipt “ button and the “Download License” button to download the payment receipt and Replaced Private Funeral Home Institution Licence respectively.

   • Print the receipt and replaced license and keep them for reference.

• Licensed Private Funeral Home Institutions in Kenya, which have lost/damaged their annual license are eligible to make the replacement application.

• Replacement fees is Ksh. 2,000

Validity

• License is valid for a period of one year.

Need for the Document

• A private funeral home institution needs to be registered with the Kenya medical practitioners and dentist council in order to be licensed to carry out its operation in Kenya.
• A private funeral home institution cannot operate without a license.

• The license issued is to be displayed in a conspicuous place at the health institution to which the license relates.
• The license issued shall specify the nature of services that may be provided.
• No premise shall be used by any person as a private funeral home institution unless it is registered and licensed for such use by the Council.
