In Person
- Visit the Office of Vital Statistics located at 275 E. Main St. in Frankfort between the hours of 8 a.m. - 3:30 p.m. EST, Monday-Friday.
- Complete an application form.
- Submit the completed form and present valid identification.
- Pay the fee.
By Mail
- Complete an application.
- Enclose a check or money order payable (NO CASH) to the Kentucky State Treasurer to cover the cost of each certificate ordered and mail to:

Copies of death certificates may only be issued to immediate family members or to persons with a legal right to the certificate.

- Additional Copies: $6 each
An additional fee is charged by VitalChek for using their service and all major credit cards are accepted.
Office of Vital Statistics (OVS)
Accepted forms of Payment:
- Cash
- Check or money order made payable to the Kentucky State Treasurer.
- The Office of Vital Statistics DOES NOT accept credit card orders by phone EXCEPT for customers requesting emergency (emergency to be defined by OVS and VitalChek) expedited delivery or genealogy records (births prior to 1911). Accepted credit card payments:

- All other credit card requests must go through either the VitalChek Call Center at 1-800-241-8322 or the VitalChek website
Notes: The Office of Vital Statistics will accept one order per customer per payment. An order consists of 20 or less requests for various certificates.

Processing TimeEdit
In Person or Phone Orders (OVS):
- Ten or less requests per order of various certificates: May be processed on the same day received subject to workload and staffing. Note: Any orders received after 3:30 p.m. EST will be processed the next business day.
- More than ten requests: There is no guarantee that your certificates will be available on the same day. Delivery of certificate is based on availability of staff and resources, OVS will strive to accommodate orders in excess of ten requests within 5 business days.
By Mail (OVS):
- In most cases, orders are filled within five to seven business days. However, some orders may take longer; therefore, please allow up to 30 working days for processing of all type of certificates ordered through the mail.
- Orders will be shipped to one delivery address
InstructionsEdit
Kentucky's Office of Vital Statistics does not have records of births or deaths prior to 1911, except delayed records of births for those born before 1911 which have been established by affidavits and documentary evidence.
You may request for copies of a certificate in person, by mail, online, by phone, or by fax.
Note: Birth, Death, Marriage and Divorce Certificates, certified copies of each of these are only available through the State Office of Vital Statistics in Frankfort.

- The county of death
- Name and address where the certificate is to be mailed.
- Phone number where you can be reached during the day from 8 a.m. - 4:30 p.m. Eastern time.
- The number of copies being ordered.
Need for the DocumentEdit
A Death Certificate is a document issued by a registrar of vital statistics that declares the date, location and cause of a person's death.
A copy of the death certificate is often necessary when you execute the will or make funeral arrangements.
Following are procedures on how you can obtain a death certificate in Kentucky.
