Procedure

1. Submit the request to the correction Office in the headquarters of the department by the relevant person himself or a family member representative to those who are outside the Kingdom.
2. Attach the documents to the application and accept the request.
3. Register the application in a special register.
4. The applicant will be assigned to bring the details of the "approved basis" in the family name registration through the civil registration file and the passport file.
5. Address the security authorities.
6. Approval of the security authorities.
7. After the issuance of the Commission's decision to approve the correction, the applicant is assigned to bring all documents issued by the Department on behalf of the former family name to be canceled.
8. Present the request to the patch panel and the issuance of its decision to grant or reject the approval.
9. Issuance of the decision for correction and inform the citizen with an official letter.
10. Bring documents issued in the name of the former family to be canceled.
11. Renew family name according to the requests submitted by the applicant in any office of the department.
12. The request is supported by official documents (birth certificate, previous passport, marriage certificate, etc.) that support the application with the correct name, whether the documents belongs to the same person or to his father or grandfather or his brothers, etc.
13. Pay the legal fees of ten JOD.
14. Issue the commissions decision (approval or rejection).
15. Implement the Commission's decision in any office of the department.
16. The applicants documents are renewed with his new family name as per his request and legal fees assessed any office of the department.
