Apply In-Person

  1. If you want to apply to change personal details under the National Insurance Scheme (NIS), you have to submit your application to the NIS office under the Ministry of Labour and Social Security (MLSS). please view the NIS contact details in the “Offices and Contact” section below
  2. Obtain the application form from the NIS office in person or download the form from the MLSS website through this link:Link (choose “Personal Details Change Form” and download the form)
  3. Read the instruction provided and complete the application form as directed
  4. Prepare the required supporting documents and attach them to the application form
  5. Once your application form and all supporting information and documents have been completed, signed, stamped, and certified where required, go to the NIS office and submit the application to the NIS official at the service desk
  6. The NIS will verify your application and determine if the application is complete as required
  7. Once everything has been ascertained and the application is successful, the NIS will implement the changes you have requested and update their system
  8. You will then be issued the relevant documents proving that the change has been implemented


Note:

  • You should follow the rules provided under “Witness’ Certificate and Signature” (Section B) then have the complete form authenticate and signed by a justice of the peace or Notary Public,Attorney, Medical Practitioner, Ministry of Labour and Social Security Parish Manager, or National Insurance Administrator
