Apply In-Person
- To apply for the National Health Fund (NHF) benefits as an individual, you should submit your complete application to the NHF office or at any card centre location. Please view the NHF contact details in the “Office Locations & Contacts” section below
- Obtain the NHF Application Form from your doctor, hospital, health clinic, pharmacy, NHF Head Office or NHF Helpdesk locations.
- Complete the application form correctly and take it to your doctor to endorse the application and fill the necessary section for doctors
- Then submit the application form to the NHF office or at any card center for processing
- Upon submission of the application, the NHF officials will verify the information you have provided in the application form to check if the form is completed accordingly
- Once they confirm that your application meets the required standard, the application will be processed and you will be registered for NHF individual benefits as per the prescribed illness
- You will then be issued an NHF registration card
