Procedure

Apply In Person

1. To change a Name (Deed Poll) in Indiana, the applicant has to approach the local Federal courts link
2. The applicant has to fill the form with the necessary details to change the name.
3. The applicant has to sign in the affidavit that he/she are not a convicted felon or have current outstanding warrant.

   Note: Don’t sign the form until the applicant bring it to a notary public who will verify the identity and witness of the applicant’s signature.

4. Submit the completed form with the required documents to the court clerk.
5. Make sure that the applicant should have all the documents mentioned in the “Required Documents” section of this page.
6. Fee for this procedure has to be paid as per the authority’s request.
7. The clerk will then date stamp the petition and enter it into the public records and return a copy to the applicant.
8. Fill out a proposal order for the judge /magistrate to sign and then the clerk will schedule the applicant for a hearing.
9. The judge/magistrate presiding the case may ask the applicant a question like reasons of changing their name, and if the is no objection, the judge will issue the certificate.
