ProcedureEdit

  1. To apply for an adoption certificate visit your local Health and Public Sanitation Department.
  2. An employee at the Health and Public Sanitation Department will provide you an application which you must then complete.
  3. Hand in the completed form to the Competent Employee of Health and Public Sanitation Department of the respective Prefecture Local Government together with the required supporting documents.
  4. The information that you have provided will be verified and your supporting documents will be checked, in case something is missing or is incomplete, you will be immediately notified.
  5. Undergo medical examinations that will confirm that you are healthy, physically and mentally.
  6. Visit your local Health and Public Sanitation Department again to receive the final adoption certificate.

  • 2 Photos
  • Completed application form
  • Chest x-ray
  • Medical Exams (diabetes, VDRL, urine test, hepatitis B and C antibodies, HIV I, II)
  • Medical certificates of neurologist, psychiatrist, venereologist, dermatologist, special pathologist (issued by the hospitals)


Note: Based on the stipulations of article 2 of the P.D. 226/99 (GG 190/?`/20-9-99) all documents required for the preparation and execution of adoptions, are defined by the Head of the service or the organisation to which the relevant application is submitted and pertain to the specified case each time.
