Apply In-Person:

  1. To Replace Lost or Damaged Community Based Organisation Registration Certificate (CBO) in Ghana, the applicant must go to the non-Profit Organization secretariat.
  2. Information related to the MOGCSP and the License authority can be obtained from the “Office locations and contacts” section of this page. contact link
  3. An applicant should start by first visiting a police station to report the loss or damage of the certificate of registration.
  4. After reporting the loss or damage, the applicant is also required to acquire a police abstract from the police authorities which is a prerequisite to making the application.
  5. An applicant is also to swear an affidavit of the circumstances surrounding the loss/ damage of the certificate.
  6. An applicant is to attach the police abstract and affidavit with the application for replacement.
  7. The minutes of the meeting seeking replacement of the lost damaged certificate, this should be endorsed by at least 2/3 of the members.
  8. Visit the non-Profit Organization secretariat and the applicant must obtain the application form.
  9. Applicant must fill all the information in the application form.
  10. The applicant has to make sure that he or she has all the required documents that are required for this process to be successful and they can be found under the "Required Documents" section of this page.
  11. Payment of appropriate fees should be made and that can be found under the "Fees" section of this page.
  12. Attach all the documents to the forms before submission application to the authority.
  13. It takes 2 working days for the Renewal Replacement to be approved and a Certificate of Registration will be issued by the MOGCSP Office.

  • A letter to the Registering Authority detailing the Reasons for seeking Replacement of the Certificate. The letter MUST be countersigned by the Assistant Chief /Chief of the Location/ Division or an official of the Social Development Committee.
  • Minutes declaring the Loss and Reasons for replacement Endorsed by at least1/2 of the members MUST be Attached to the Application form
  • A Police Abstract.
  • Attachment of a portion of the original certificate/photocopy where Available
  • Replacement fees receipt.

ValidityEdit

  • The lost or damaged its registration certificate validity is for 1 year. Then, the certificate has to be renewed annually.
