Apply In-Person:
- To Replace (Lost,Stolen,Damaged,Duplicate) National Health Insurance (NHIS) Card, the applicant has to approach the National Health Insurance Authority link
- An application form will be provided to the applicant by the concerned.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach photocopies of the required documents along with the application form while submitting it.
- Submit all the documents with the completed form to the concerned officer.
- The applicant has to pay the cost of replacing a lost, stolen, damaged, or duplicate NHIS card is Ghana cedi 10.00.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
- The applicant might be required to provide new passport-sized photos for the replacement NHIS card.
- Confirm the photo requirements with the NHIS office.
- Once the application is approved and processed, they be informed about when and how to collect the applicant replacement NHIS card.
- This entire process can be completed within 2 weeks or as per the timeline advised by the respective department authority.

- Proof of residency (e.g., utility bill, tenancy agreement).
- Police report (if the applicant’s card was stolen)

- The applicant has lost his/her NHIS card.
- The applicant's NHIS card has been stolen.
- The applicant's NHIS card is damaged or illegible.
- The applicant needs a duplicate NHIS card.
