Ghana - How to Replacement of Lost O and A Level Certificate

Apply In-Person:

  1. To Replacement of Lost O and A Level Certificate, the applicant has to approach the Ghana Education Service (GES) Headquarters.
  2. Address and contact details can be found in the following Contact link.
  3. Applicant must obtain the application form from the relevant department.
  4. Fill in the application form accurately and attach all the required documents and they can be found under the "Required Documents" section of this page.
  5. Payment of appropriate fees should be made and that can be found under the "Fees" section of this page.
  6. Submit the completed form along with the supporting documents to the respective office.
  7. This application will be processed and information will be verified by the GES.
  8. If the verification is successful, the notification will be sent to the applicant.
  9. The applicant after seeing the notification can go to the GES office and the applicants get their replacement certificate.
  10. Maximum Processing Time for this procedure is 5 days.

  • All candidates who sat for their Ghana national examinations and lost or damage their O and A level certificate.



FeesEdit

  • 50,000 GHS for an Ordinary Letter and 100,000 GHS for a Hard Card.
