Procedure

Apply In-Person:

1. For Correction of Information on an ID card in person, the applicant must visit the Ghana National Identification Authority. Contact link
2. Applicant must obtain the application form from the relevant department.
3. Read carefully through the application form as there contain different section.
4. The applicant has to make sure he or she has all the required documents that are required for this process to be successful and they can be found under the "Required Documents" section of this page.
5. Payment of appropriate fees should be made and that can be found under the "Fees" section of this page.
6. Submit the completed form in two copies along with the supporting documents to the respective office.
7. This application will be processed and information will be verified by the NIA.
8. If all the above mentioned procedures were followed properly by the applicant, the application form will be viewed by concerned authorities and the applicant will be notified regarding their license.
9. The applicant after seeing the notification can go to the NIA office and applicant get the certificate.
10. The Processing time for this procedure is 1 month.

Required Documents

Change name in ID card

• A deed poll (which can be acquired through a lawyer and be approved by Ghana Registration Services Bureau.
• Copy of Ghana gazette add of change of name
• Copy Identification document e.g. birth certificate, marriage certificate
• Accurately dulled filled application form
• Bank slip (proof of payment)

Correcting spelling error

• Copy of Supporting documents e.g. birth certificate, marriage certificate, academic document, baptism certificate, work document etc.
• A statutory declaration duly registered by Ghana Registration Services Bureau.
• Accurately filled application form
• Bank slip (proof of payment)

Changing place of residence

• A letter from the L.C.I chairperson, duly stamped by the DISO (including telephone contacts), confirming citizenship, current residence and the period of time applicant has stayed at the said residence.
• Accurately filled application form
• Bank slip (proof of payment)

Required Information

• Name of the applicant
• Date of birth of the applicant
• National identification number
• Full name of Applicant preferred new name
• Current full name of the applicant

Need for the Document

• The identification card is used to prove one is a Ghana citizen, and all Ghanaian citizen of 18 years and above are required to have a valid ID card.

Information which might help

• To correct information on your national ID, you’ll be charged UGX 50,000. (The money is paid at any URA revenue collection bank such as Bank of Ghana, Diamond Trust Bank, Stannic, Barclays, Orient, etc. [Remember, the fees are exclusive of bank charges].)
