Procedure

Apply In-Person

1. To Apply for Private Institution Medical License in Ghana, the applicant must go to the Ghana Medical and Dental council or Pharmacy Council Ghana, Headquarters link
2. Application form can be downloaded from the Link or Visit the office and obtain an application form to be completed for Private Medical Institution License with respect to the requirement, from the relevant department.
3. Complete the form with appropriate information under the relevant sections. Attest the forms with the applicant’s signature once completed.
4. Once completed filling the application form, make sure that The applicant attached all the documents listed in the “Required Documents” section of this page.
5. Payment of appropriate fees should be made and that can be found under the "Fees" section of this page.
6. Then, submit it in person to the relevant authority at the office. Make the stipulated payment as directed by the officials.
7. After submission, the application and other documents will be forwarded for examination and verification processes.
8. An inspection may also be conducted at the institution to be registered, by the officials from MDC.
9. Once all the necessary processes are over and if the application is found sufficient, it will be approved and the officials will proceed to take required steps to issue the Annual license.
10. When the license is ready to be collected, the applicant will receive a notification from the office through email or phone call regarding the collection and the applicant can collect them by any applicable means.

• Educational Qualification certificates (12th, all years mark sheet of D. Pharmacy, B. Pharmacy or Pharmacy D)
• Proof of Identification and Proof of residence in state applying for registration
• Date of Birth Proof
• Practical training certificate (For D. Pharmacy students only)
• Prescribed fee
• Photographs
• Affidavit if required
• Original character certificate / College Leaving Certificate/Migration /transfer certificate
• Letter from pharmacy institution/college showing its approval from PCI for the year in which candidate qualifies for its diploma/degree.
• Diploma Degree certificate
• Any other document asked by state pharmacy council
