Obtain Police Abstract Report

  • The applicant should start by first visiting a police station to report the loss or damage of the certificate of registration.
  • After reporting the loss or damage you are also to acquire a police abstract from the police authorities which is a prerequisite to making the application.
  • You are also to swear an affidavit of the circumstances surrounding the loss of the certificate.
  • You are to attach the police abstract and affidavit with your application for replacement.


Apply In-Person:

  1. Replace Lost/ Damaged Self-Help Group (Community Project) Registration Certificate, the applicant has to approach Ministry of Labor and Social Affairs in Ethiopia contact link
  2. Applicant has to get the application form to renewal from the respective office.
  3. Applicant must provide all necessary information in the application form.
  4. Make sure you have all the documents mentioned in “Required Documents” section of this page.
  5. Complete the application form and attach the required document.
  6. Submit the completed application form along with the required documents to the authority.
  7. Fee for this procedure has to paid as per the authority request.
  8. Upon the successful registration, the renewal of license will be issued within 2days.

  • List of All the Members duly signed with Name/Position/ID No./Telephone No. and Signatures MUST be attached to the replacement Form
  • Constitution
  • Minutes declaring the loss/reasons for replacement, this should be endorsed by at least 2/3 of the members.
  • A Police Abstract.
  • Attachment of a portion of the original certificate/photocopy where available.
