Procedure

Apply In-Person:

1. The application form for obtaining a birth certificate is available in the Ministry of Health, Ethiopia contact link
2. Applicant should fill-in all the necessary details that are asked in the application form.
3. Applicant should make sure to carry all the documents that are mentioned in the “Required Documents” section of this page (Both photocopies and Originals).
4. Applicant must attach the photocopies of the documents (whichever is necessary) along with the application form.
5. The issuance of a local birth certificate costs approximately $2. Expeditious or same-day service costs approximately $5 more.To have a birth certificate or other document authenticated at the Ministry of Foreign Affairs costs approximately $16 for Ethiopian parents and $35 for foreign parents.
6. If the applicant follows all the above-mentioned procedures properly, then the birth certificate will be issued to the applicant within a fortnight.
7. If the applicant doesn’t get any information regarding the permit within a month, then, the applicant has to visit the office once again to get details on their application form.

Required Documents

The documents that is necessary for the procedure is listed below:
For children less than ten years of age

• Hospital birth certificate.
• Court document confirming parentage, or a baptismal certificate.
• Acknowledgment from one’s “kebele” (urban neighbourhood association).
  

For children between the ages of eleven and thirteen
The above mentioned documents and school records are required for issuance.Any of the above documents and an identity card is satisfactory for issuance of birth certificates for persons over the age of eighteen.

Instructions

• Persons applying for original birth certificates should contact the appropriate municipal authorities and present hospital birth records, which have been authenticated by their "kebele (local government office).Because of the nature of the "kebele" system, the birth certificate may be issued by the municipality where the person presently resides rather than where the birth actually took place.
• Birth certificates are available only to those resident in Ethiopia. Requests for copies of previously issued birth certificates should be addressed to the Office of Region 14 Administration, Vital Statistics Service, Municipality of Addis Ababa, P.O. Box 356, Addis Ababa, Ethiopia; or to the equivalent office or any other municipality in which the birth was registered.

Need for the Document

An applicant wishing to obtain a birth certificate must contact a city hall in Ethiopia. The applicant needs to arrange for a family member or friend in Ethiopia to obtain the birth certificate on their behalf from the city hall. If the applicant does not have a family member or friend in Ethiopia, the Ethiopian embassy can provide the applicant with the telephone number and mailing address of the appropriate city hall so that the applicant can make direct contact with the city hall.
