Procedure

Apply In Person

1. To Obtain Licence or Permit for Food Handling Premises, the applicant has to approach the Ministry of Food Drug Administration link
2. The applicant has to get the application form from the food and drug administration department to get the food handling premises license.
3. Applicant has to fill-in all the necessary details in the licensing form.
4. Make sure that you have all the documents listed in the “Required Documents” section of this page.
5. Submit the completed application form to the respective officer with the necessary documents.
6. Fee for this procedure has to paid as per the authority request.
7. The office tests the food handling and the documents with the given form.
8. Once the process is completed, the license is given to the applicant within the stipulated processing time period of time.

• Plan of the processing unit showing the dimensions and operation-wise area allocation
• List of Directors/ Partners/ Proprietor with address, contact details, and photo ID
• Name and list of equipment and machinery used with the number and installed capacity
• List of food category to be manufactured
• Authority letter from manufacturer nominated a responsible person name and address
• Analysis report of water to be used in the process to confirm the portability
• Source of raw material for milk, meat etc.
• Proof of possession of premises
• Partnership deed/ affidavit of proprietorship
• NOC and copy of License from the manufacturer
• Food safety management system plan or certificate
• NOC from the municipality or local body
• Supporting document for proof of turnover and transportation
• Declaration form
